Page 1 is for the Purchaser/Lessee. Please type in all requested information.
If applying for joint credit with a spouse or partner or if you are cosigning for someone, then you will also need to complete their information in the lower half of the application on the same page with the heading “CO-PURCASE-LESSEE”.
Page 2 is not for California residences but does state that a California residence may apply for a separate account if married.
Page 3 please read and sign next to the x under ‘signature of applicant’. Then check the square right below your signature that says Broker arranged transaction yes and Lease Factor as the Broker name.
Once completed, sign the bottom of page 2.

Print the application and complete pages 1, 2, and 3.
Page 1 is to be completed with information pertaining to business. The bottom of page should list the operator of the car, (this is not necessarily the financial responsible person)
Page 2 is to be completed with information pertaining to the personal guarantor (this is the person who will be financially responsible for the payments of the car through the business)
Page 2 and 3 shall contain signatures where indicated at the bottom of the pages
